Facility Evaluation Report

Licensing Program Analyst (LPA) David Marrufo conducted a Case Management Visit and met with Scott and Juana Guerin, licensees. The purpose of the visit was to follow up with the Certificate of Non-Operation that the licensees signed on 09/29/2025 and that the department received on 10/09/2025.

During visit, LPA Marrufo toured the facility. LPA Marrufo observed that the three out of three bedrooms did not have any belongings that belonged to any residents. There were no residents at the facility during visit.

During telephone call on 10/13/2025, Licensee Juana Guerin stated that the facility had closed 02/15/2025.

On 10/15/2025, LPA Marrufo received an email from Licensee Scott Guerin that contained copies of the letters given to the last remaining three residents. The letters stated the residents were being given a 90-day notice that the facility would close on 02/15/2025.

A deficiency was cited as per California Code of Regulations Title 22. See LIC809-D page for more information.

This report was reviewed with Licensees Scott and Juana Guerin and a copy of this report and appeal rights were provided.
