Facility Evaluation Report
On 3/5/2026, Licensing Program Analyst (LPA) Grace Donato conducted an unanncouned case management - incident visit in response to a report received through a complaint. LPA met with Executive Director Riley Tucker & Nick Catalano.
In reference to complaint report 26-AS-20240619104312, it was stated by the reporting party (RP) that he/she has seen staff yell at residents when resident say they want something to eat during a non meal time.
LPA interviewed 5 staff members and all mentioned that they have not seen staff yell at residents. S1 stated that staff voice gets loud when talking to some residents because these residents have issues with hearing. They have to speak a bit louder so their residents are able to hear them. S2 & S3 also confirmed that they do have residents who are not able to hear well so they have to come close to them so they'd be able to understand what they are saying.
Based on these interviews, staff are not yelling but just talks loudly so the residents are able to hear what they are saying.
No deficiencies cited today. Report is reviewed and copy is provided.
