Facility Evaluation Report
On 4/24/2024 at 9:20 a.m. Licensing Program Analyst (LPA) Maria (Mita) Partoza arrived and conducted an unannounced required annual visit and met with Program Director David Lopez and Program Manager Alisha Roswall.
LPA toured the facility and observed the following no toxic substance was observed accessible to the clients. The room temperature measured at 71 degree F. Hot water temperature measured at 106.3 to 107.2 degrees F. Both men and women's bathrooms floor observed to be dry, sanitary, and free of hazard. The classrooms and activity room were observed sanitary and free of odors. Emergency exits were well-lit and free of obstruction. The facility fire alarm is monitored by the Department of Public Safety Fire Prevention and Hazardous Materials Certified Unified Program Agency, City of Sunnyvale for the entire property. The fire system was tested on March 4, 2024 and was found to be in good working condition. Fire extinguisher was last inspected on 9/15/2023.
The facility uses one vehicle to transport clients. The vehicle maintenance record was reviewed and was last serviced on 1/23/2024. First aid kit was inspected and found to have all the necessary first aid tools, cream and bandages.
LPAs reviewed 4 out of 45 in person participant files and 4 out of 12 staff files. All clients' files reviewed contained current needs and services plans and Individual Program Plans. All staff have criminal background clearance from Community Care Licensing and are associated to the facility. Health screening and current first aid certification. Individual staff training are up to date. Facility's disaster and fire drill training conducted every quarter most recent training was done on 2/13/2024.
