Facility Evaluation Report
Licensing Program Analyst Simi Rai arrived at the facility unannounced to conduct a case management – other visit. LPAs met with Administrator, Darlington Objegbu. LPAs observed 2 staff members and 3 residents at the facility.
The purpose of the visit is to hand deliver a Criminal Record Exemption Needed letter for an individual (S1) who the Department determined this individual must be immediately removed from the facility.
Per Administrator, Darlington Objegbu S1 was not currently working in the facility and Criminal Record Exemption Needed letter was completed and submitted to the Department.
The Criminal Record Exemption Needed letter was handed to the Administrator, Darlington Objegbu.Administrator. Darlington Objegbu was informed to remove S1 from any contact with residents and not allow S1 to be physically present in the facility.
No deficiencies were cited per California Code of Regulations, Title 22.
This report was reviewed with Administrator, Darlington Objegbu and a copy of the report was provided.
