Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Required 1 Year visit and met with Xyline Mendoza-Catli, Program Director. LPA announced the purpose of the visit. LPA observed clients doing activities. The facility has 4 rooms and separate cubicles for residents to do various activities including computer technology room, as well as arts and crafts.
During visit, LPA toured the facility inside and out. LPA observed locked cabinet for PRN medication and a First Aid Cabinet. LPA toured the kitchen area and observed locked cabinets for sharp objects, and cleaning supplies. The facility does not prepare any food, clients bring their own food. The facility has emergency food supply. The facility has a fire pull alarm system and sprinkler system that was last serviced on 04/14/26.
LPA toured client bathrooms each bathroom had soap and paper towels. The water temperatures in the bathroom sinks measured with thermometer at 109.4 to 115.5 degrees Fahrenheit.
LPA toured the outside area and found the exits to be clear of obstructions. LPA observed fire extinguisher was last serviced on 02/12/26. LPA reviewed Fire and Earthquake log. The facility conducted last fire drill on 05/19/26 and earthquake drill on 05/26/26.
LPA reviewed records for 5 clients. LPA reviewed 5 staff records. The facility has 8 transportation vans that are maintained and serviced regularly. The facility does not handle cash resources.
No deficiency were cited as per California Code of Regulations Title 22. This report was reviewed with Xyline Mendoza-Catli and a copy of this report was provided.
