Facility Evaluation Report
Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection, and met with Executive Director (ED) Josh Drake and Program Director (PD) Aya Sasaki.
LPA toured the facility inside out including: Main activity room that was partitioned into 4 activity sections, upper activity room, 4 common restrooms, kitchen, office area, side yard and garden. Dish washing solution bottle was observed on the kitchen counter. PD locked it immediately. Butter knives in the drawer were observed unlocked. PD locked the drawer immediately. Two refrigerators were observed. Temperatures were observed at 37 degrees F and Freezer temperature was 0 Degrees F. One storage room was observed by the garden. There was no obstruction to block any outdoor exits. plies. LPA observed Facility License and Personal rights were posted.
LPA observed the medication storage area, and cleaning products were locked and inaccessible to clients in care. Room temperature was at 68 degrees F. LPA inspected the facility first aid kit and it was observed to be complete. The facility was equipped with smoke and carbon monoxide detectors. PD tested the carbon monoxide detector, and it worked. Fire extinguishers were last serviced on 1/1/25. The facility conducted their last fire drill on 10/2/25. LPA reviewed facility files for 5 staff and 6 clients.
No deficiencies were cited during today's visit as per California Code of Regulations Title 22. Exit interview was conducted with PD. This report was reviewed and a copy was provided to PD.
