Complaint Investigation Report

2 staff and Administrator: Denies all allegations. 1 Witness affirmed all allegations.

Records reviewed revealed that medications are documented and there’s no gap in service. Physician Report indicated that resident has a history of verbal abuse. Menu revealed that facility offers up a variety of different nutrition on weekly basis. No records of eviction. Interviewed with staff indicated that temperature is 70 F and is regulated on an automatic setting. No reports or documentation of verbal inappropriateness.

Based on information from interviews conducted and records reviewed, although the allegation listed above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are UNSUBSTANTIATED.

No Deficiencies cited under California Code of Regulations Title 22.
