Facility Evaluation Report
Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced case management visit to follow up on the incident that occurred on 3/19/2025 and met with Administrator Jocelyn Perez. LPA stated the purpose of the visit.
On 3/20/2025, the Department conducted an initial case management for the incident and requested for documents regarding resident 1 (R1).
During today's visit LPA toured the facility inside and outside, including but not limited to the living and dining room, kitchen, 4 resident bedrooms and 2 bathrooms. LPA observed a chicken coop in the backyard.
LPA interviewed ADM and requested for additional information regarding the incident that occurred on 3/19/2025. ADM stated the facility will update the Department by 7/4/2025.
No deficiencies were cited during today's visit based on California Code of Regulations Title 22. An exit interview was conducted with Administrator Jocelyn Perez and a copy of the report was provided.
