Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced case management visit to follow up on the incident that occurred on 3/19/2025 and met with Licensees and Administrators Marilou Williams and Jocelyn Perez.
On 3/20/2025, the Department conducted an initial case management for the incident and requested for documents regarding resident 1 (R1).
During today's visit LPA toured the facility inside and outside, including but not limited to the living and dining room, kitchen, garage, 4 resident bedrooms, den, and 2 bathroom. LPA observed smoke and carbon monoxide detectors. LPA observed a chicken coop in the backyard.
LPA conducted an interview with staff and licensees and requested for additional information regarding the incident that occurred on 3/19/2025.
No deficiencies were cited during today's visit based on CCR Title 22. An exit interview was conducted with Licensee and Administrator Marilou Williams and Jocelyn Perez and a copy of the report was provided.
