Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit. LPA announced the purpose of the visit. LPA met with Administrator Anthony Tran and Staff Thuy Tran.

LPA conducted a case management visit regarding an incident that occurred with on 05/19/26 regarding Resident (R1).

ADM stated R1s Physicians report states he/she is able to leave unassisted and the day of the incident R1 was crossing the street and was hit by a vehicle and suffered bruising and fractured finger. ADM stated when R1 was run over that R1 was on his/her way home. ADM stated that R1 had not returned home and they were worried and began to call authorities when they received a call from local law enforcement.

ADM and staff stated R1 is able to leave unassisted and communicate with him/her using cellular phone and text to make sure R1 is ok.

During visit LPA interviewed ADM and Staff. ADM stated SARC coordinator stated R1 is being assigned a Housing Choices Case Manager.

ADM stated they are working with R1 to get him/her to attend meetings for substance abuse. R1 is currently court mandated to attend meetings for Substance Abuse. LPA requested copy of R1s Appraisal needs and Services.

No deficiencies were cited per California Code of Regulations, Title 22.

This report was reviewed with Administrator, Anthony Tran and copy of the report was provided
