Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced 1 year required visit. LPA announced the purpose of the visit. LPA observed 2 staff and 6 residents. LPA met with Designated Administrator (DA) Iluminada Yap.
During visit LPA toured the kitchen and observed locked cabinet for cleaning supplies and Sharp objects. LPA observed perishable food supply of at least two days and a non-perishable food supply of at least seven days. Refrigerator temperature measured with thermometer at 42 degrees F and freezer at 0 degrees F.
LPA toured 4 resident bedrooms and observed room with bedding, dresser, and lighting. LPA toured 2 resident bathrooms, and 1 staff bathroom. Bathrooms water temperature measured with thermometer between 114-119 degrees F.
LPA observed a locked cabinet located in kitchen where residents medication, resident records along with staff files are kept. During tour LPA observed the hallway had hand rails for residents to use. The facility has a staff bedroom.
LPA toured the facility inside and out and observed walkways free and clear of obstruction. LPA observed ramp leading from hallway to backyard. LPA observed a locked shed being used as storage and not living quarters.
LPA observed fire extinguisher was last serviced on 01/7/26. LPA reviewed the facility disaster drill log and the last disaster drill was conducted on 03/02/26.
