The inspector’s account

Licensing Program Analyst (LPA) Steve Chang Conducted an unannounced Plan of Correction visit to follow up the case management conducted on 2/04/2025, and met with Administrator (ADM) Patrick Laurel.

The facility was cited as Title 22 section 82078(a) Responsibility for Providing Care and Supervision . On 1/28/2025, Client C1 left the facility without staff knowledge and found by staff 1.7 miles away from the facility.

LPA checked the door alarm system with ADM for the 5 exit doors. The alarm system only notifies the front desk and the volume of the alarm is not loud. ADM stated the facility is to install secondary alarm for each exit door to make the volume of the alarm loud enough to be easier to get the attention of the staff.

LPA discussed resident R1's updated Appraisal Needs and Service plan with ADM. LPA discussed the facility policy of transpiration of clients with ADM. LPA discussed with ADM regarding the facility protocol for clients to go to restroom. LPA discussed the policy for the inside facility activities and out side facility activities with ADM. LPA checked the physician reports and appraisal needs and service plan of clients with ADM. LPA discussed the facility staffing with ADM. LPA discussed with ADM regarding the facility policy for staff call in sick. LPA discussed with ADM about the staff training of elopement. LPA reviewed the staff training log.

Exit interview was conducted with ADM. The report was provided to ADM for review and signature. A copy of the report was provided to ADM.
