The inspector’s account

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Administrator (ADM) Patrick Laurel and Lead Staff Marta Malapitan (S1).

LPA reviewed 8 client files and 8 staff files.

License and personal rights poster were observed at the main entrance. LPA toured the whole facility with ADM.

Lobby, front desk, offices, conference room, classrooms, library, movie room, activity rooms, restrooms, kitchens, and break room were observed and inspected. Dish washing solution bottles were observed on the top of the sinks in the kitchen, ADM put the dish washing solution bottles in the cabinet under the sink in the kitchen and locked it immediately.

Hot water temperature was measured at 115 degree F. Room temperature was 70 degree F. The facility was equipped with smoke and carbon monoxide detectors. Smoke detector alarm system and carbon monoxide detectors were tested, and were working fine. Fire extinguisher was serviced on 5/30/2024. Fist aid box and emergency light system were observed in the facility.

The last time the facility conducted the fire drill was on 9/10/2024, and the earthquake drill was conducted on 10/17/2024.

Front yard and backyard were inspected. There was no obstruction to block the walkways.

Exit interview was conducted with ADM, No citation was noted for today visit. This report was provided to ADM for signature. A copy of the report was provided to ADM.
