Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit. LPA announced the purpose of the visit. LPA met with Prunella Cardozo, Administrator.

During visit LPA was provided a copy of an Eviction notice for Resident (R1). The eviction notice was dated 06/10/26 for R1 needing a higher level of care. ADM did provide R1 the eviction notice on 06/12/26 for a 30 day eviction date of 07/10/26.

ADM was informed that this is not a lawful eviction because it is not 30 days. ADM did not provide the department a signed copy of the eviction notice within the 5 days from serving resident the eviction.

ADM and LPA also discussed the importance of ADM to follow regulations regarding eviction procedures that were also discussed during a previous visit on 10/29/25.

Technical Violation was issued please see LIC9102. Deficiency was cited during todays visit see LIC809-D.
