Facility Evaluation Report

Licensing Program Analyst Marcela Yanez conducted an unannounced case management visit. LPA met with Administrator (ADM) Prunella Cardozo and announced the purpose of the visit.

During visit LPA interviewed ADM regarding incident that occurred 10/15/25, where a male resident approached 2 different female residents and verbalizing if they wanted to have sex.

ADM stated that the resident was given a warning of house rule #18 (Resident shall not harass another Resident) and Rule #16 (resident will respect other residents privacy)

ADM stated the resident is currently at EPS (Emergency Psychiatric Services) on a 5150 hold.

LPA Yanez determined the incident does not need further investigation.

An exit interview was conducted with ADM and a copy of report was provided. no deficiencies cited.
