Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced case management visit and met with Program Administrator (ADM) Jennifer Murillo. LPA addressed to ADM that the purpose of today's visit is because the Department received a report from DDS and LPA is to follow up.

LPA observed 2 staff (S1, S2) and 1 resident (R1). The other 2 residents went to day program.

On 10/04/2025, the Department received an on site visit report from Department of Developmental Service.

LPA toured the facility inside and out with S1 including office, living room, small activity room, kitchen, 3 bedrooms, 1 storage room, 2 bathrooms and backyard..

LPA inspected the ceiling exhaust fan in the bathroom, the transition strip on the floor leading from the dining room to the kitchen, and the top of the archway in the dining room .The 3 deficiencies are observed fixed.

ADM stated on 10/08/2025, two LPAS already conducted a case management and observed the fix. ADM show the LIC809 dated 10/08/2025.

Exit interview was conducted with ADM. The report was provided to ADM for review. A copy of the report was provide to ADM.
