Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management Visit LPA met with Jennifer Murillo Program, Administator (ADM) and David Sandhu Director Of Operations. LPA announced the purpose of the visit. LPA observed 1 resident and 1 staff.

During visit LPA toured the facility inside and out. LPA toured 3 resident bedrooms, living room, backyard and kitchen area. During visit LPA observed a 2x1 inch area where plaster is missing from the entry way to the living room/activity room it does not appear as peeling it is missing piece of plaster, the maintenance person was on vacation just returned on 10/08/25.

During visit LPA observed the vent fan in the bathroom #1 to be working and in good repair and it is a very quiet fan and is working.

ADM stated that the facility is leased and a work order was submitted to the owner of the property and are waiting for a repair appointment for the 2x1 inch plaster that is missing and for the floor connection the kitchen to the activity room missing a piece of 6x2 inch of wood.

No deficiency was cited per California Code of Regulations, Title 22. Advisory note provided. A plan of correction was requested for the advisory note.

This report was reviewed with Administrator, Jennifer Murillo Program Administrator , David Sandhu, Director of operations and a copy of the report was provided.
