Facility Evaluation Report
Licensing Program Analyst (LPA) Christine Kabariti arrived unannounced to conduct a POC (plan of correction) visit from a Type A deficiency that was cited on 07/25/2025 during the facility’s required 1 year annual inspection. LPA met with Administrator, Mark Castillo.
On 07/25/2025, the facility was issued a Type A deficiency on Title 22 Section 80088(e)(1) based on the observation of the hot water temperature measured at 144.5 degrees F next to room 103.
On 07/26/2025, the facility submitted their plan of correction (POC) in writing. On the POC, it was stated that the water temperature was measured on 07/25/2025 at 3:00pm. The water temperature was recorded as followed: “105 bathroom sink temp 117 degrees F, 110 shower temp 107 degrees F, 210 shower temp 122 – water heater temp was adjusted and caution sign will be posted water temp may reach 125, 212 bathroom sink next to med room temp 124 – water heater temp was adjusted and caution sign will be posted temp may reach 125”.
During today’s visit, LPA measured the hot water temperature as follows: 10:39am: Room 110 - 113.7 degrees F; 10:40am: Room 105 - 120.7 degrees F; 10:45am: Room 212 – 115.3 degrees F; 10:49am: Room 210 – 111.9 degrees F. LPA did not observe any caution signs regarding the hot water temperature reaching 125. ADM states they took down the cautions signs because the hot water is not going over 120 degrees F after measuring the water temperature weekly.
No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Administrator, Mark Castillo and Assistant Administrator, Armando Guba and a copy of the report was provided.
