Facility Evaluation Report

Licensing Program Analyst (LPA) Christine Kabariti arrived unannounced to conduct a case management - incident visit. LPA met with Administrator, Mark Castillo.

The purpose of the visit is to follow-up on 2 incident reports and 2 SOC341s the Department received on 08/13/2025.

During visit, LPA obtained the following documents: surety bond, resident roster, staff roster, 30 resident's admission agreement; physician's report; and LIC405; staff (S1)'s personnel file (LIC501, LIC508, and timecard); and 2 staff members LIC855 (Declaration).

This case management will be open pending further investigation.

This report was reviewed with Administrator, Mark Castillo and Assistant Administrator, Armando Guba and a copy of the report was provided.
