Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit and met with Cynthia Octabiano, Designated Administrator. LPA announced the purpose of the visit. LPA observed 3 staff and 4 out of 4 residents.
On 11/10/25 the department received an SOC341 alleging neglect from staff towards a resident. LPA requested documents including but not limited to Physicians Report, Appraisal needs and services as well as functional capability test.
During visit LPA interviewed DA regarding incident. DA stated that the DSP never left the line of sight of the resident when he/she was weighing the adult brief and when the resident was placed in the van. The resident does not have a 1 on 1 caregiver. DA stated there were a total of 3 staff during shift. DA stated 4 out of 4 residents do not require 1 on 1 care. DA stated that training was conducted regarding safety of resident and responding to and reporting suspected resident abuse and neglect. DA provided a copy of sign in sheet of staff that attended with date and time training was conducted.
During visit LPA observed staff constantly monitoring residents care and all staff are in the same vicinity and able to see clients from kitchen and front door.
LPA determined the above incident does not need further investigation.
