Licensing Program Analyst (LPA) Steve Chang conducted an unannounced case management visit and met with Administrator (ADM) Arielle Teodoro.
LPA interviewed ADM, a staff, and resident R1.
LPA toured R1's bedroom and facility restrooms.
LPA requested R1's physician report, appraisal needs and service plan, and discharge document.
At this time, the case is under review and the Department will conduct a follow visit if warranted.
Exit interview was conducted. The report was provided to ADM for review. A copy of the report was provided to ADM.
