Facility Evaluation Report

Licensing Program Manager (LPM) Christine Kabariti and Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit. LPM and LPA announced the purpose of the visit. LPM and LPA met with Mona Hada, Administrator, and Diane Wood, Office Manager (OM).

On 07/24/26 a case management visit was conducted to follow up on the updated facility Program Design to an "Adult Medical Day Program" after CCLD was made aware.

On 07/31/26 LPM and LPA conducted a follow up visit to get more information regarding their new program design. ADM provided a copy of individual roster, staff schedule, activities calendar for July and August 2026, and latest SARC compliance review checklist for day support program design.

During visit ADM and OM was interviewed and discussed the Exception for13 individuals that are using chest strap and/or seatbelt in wheelchair. ADM stated that their are 8 clients with restricted health conditions.

During visit LPM and LPA toured the facility and observed individuals in groups doing activities with staff. LPM and LPA observed 3 facility vehicles, including 1 bus and 2 vans.

No deficiencies were cited per California Code of Regulations, Title 22.
