Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit. LPA announced the purpose of the visit. LPA met with Mona Hada, Administrator.

The purpose of todays case management visit is to follow up on the udated facility Program Design after CCLD was made aware.

During visit LPA interviewed Administrator and obtained copies of 15 clients records including but not limited to physicians report, appraisal needs and services, Individual Program Plan (IPP), Restricted health care plan. LPA also received copies of staff training for restricted health condition for 8 individuals.

ADM also provided copies updated Program Design dated 05/22/26 and SARC checklist.

Based on interview and record review the facility did not notify CCLD of the updated facility Program Design.

Deficiency was cited as per California Code of Regulations Title 22. This report was reviewed with Mona Hada, Administrator and a copy of this report and appeal rights were provided.
