Facility Evaluation Report

Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced Collateral visit. LPA met with Administrator (ADM) Debbie Silva. LPA stated the purpose of the visit.

During the visit, LPA interviewed 1 Staff (S1) and 1 Client (C1). LPA also requested pertinent documentation to include but not limited to client physician's reports, and client service plans.

LPA is conducting a collateral visit as part of complaint investigation for Complaint # 26-AS-20260722153226.

No deficiencies were cited during today's visit, per California Code of Regulations, Title 22. An exit interview was conducted with ADM Debbie Silva and a copy of this report was provided.
