The inspector’s account

On July 10, 2026, Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced case management - incident visit regarding an incident report regarding resident R1. LPA met with staff Daniela Verar and explained the purpose of the visit.

During today's visit, staff Daniela Verar confirmed that Administrator Alison Noble is not the facility administrator and has not worked at the facility for several years. LPA informed staff Daniela Verar that the Department requested for documents to process the change in Administrator on June 29, 2026. As of July 10, 2026, the Department has not yet received the documents necessary to process the change in Administrator. LPA requested the facility submit the following documents of the prospective administrator to LPA Monter by July 15, 2026:

1.Board of resolutions letter, stating the Date the employee will begin working as Administrator and signed by the licensee, who is part of the corporation.

2.Administrators Certificate 3.LIC 501 4.Resume 5. LIC 500 6.LIC200

On June 24, 2026, the Department received an incident report (IR) regarding resident R1. The IR stated the following: on June 19, 2026, resident R1’s family member (FM) called the facility to report that R1 had a big bruise on the inside of his/her right arm. R1 told FM it happened on Thursday, wherein a staff grabbed him/her and the bruise was the result.
