Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management– legal/non-compliance visit. LPA announced the purpose of the visit and met with Regional Director (RD) Daniela Verar. The previous Program Director Michele Pagsisihan is no longer with the facility as of 11/06/25.
The purpose of the visit is to ensure the facility is adhering to the compliance plan submitted to Community Care Licensing (CCL) after a non-compliance meeting held on 12/12/2024. During visit LPA toured the facility 6 activity rooms which included activity room to play chess, art room, computer room, music room, kitchen, lunch area. and 3 bathrooms. LPA observed facility staff and clients planned and prepared a meal consisting of tacos. The facility also has a changing room, copy room, and locker room for residents to use. LPA observed a calendar on the wall with clients birthdays.
The facility has 10 vans 5 ambulatory and 5 wheelchair accessible. LPA observed 5 out of 5 wheelchair transportation van numbers 58314, 71811, 28419, 50314 with the RD which has wheelchair safety procedures and diagrams posted on the back door (Sticker wheelchair van tie down securement) (Shoulder and lap belt positioning). The facility is still coordinating a CPI training for 4 staff recently hired. RD stated the vans are serviced continuously as needed. The facility recently conducted training for best practices for activities and teaching techniques for staff on 01/13/26.
RD was advised the importance the facility adheres to the compliance plan that was submitted on 12/12/24 to ensure the facility stays within compliance of Title 22 regulation. No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Regional Director, Daniela Verar and a copy of the report was provided.
