Facility Evaluation Report

Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct a Case Management-Incident visit regarding an incident of alleged violation of personal rights. LPA met with Program Director (PD) Alejandrino Espinoza Cruz.

On 10/20/2025 the facility submitted an incident of alleged violation of personal rights that was reported by Client C1 to facility staff on 10/13/2025. The alleged incident occurred at C1's place of residence on 10/12/2025.

During today's visit, LPA Tarin interviewed Staff S1. S1 states the alleged violation of C1's personal rights occurred outside the facility at C1's place of residence.

LPA determined this case management does not require further investigation.

No deficiencies were cited during today's visit per California Code of Regulations, Title 22. An exit interview was conducted with Program Director Alejandrino Espinoza Cruz and a signed copy of this report was provided.
