Facility Evaluation Report
Licensing Program Analyst (LPA) Christine Kabariti arrived unannounced to conduct the facility's required - 1 year annual inspection. LPA met with Program Director (PD), Alejandrino Espinoza. During visit, LPA toured the facility with PD to include the main activity room, classrooms, office space, conference room, storage, dining room, kitchen, bathrooms and exterior. All fire exit routes are free and clear of obstruction. No clients observed on site as they were out in the community. Facility temperature maintained at 72 degrees F. Carbon monoxide detector present in the dining room. Fire extinguisher last serviced on 10/01/2025. Sharp objects observed locked in the kitchen area. Drinking water available in the refrigerator. Chemicals and disinfectants observed locked in a storage closet. Bathrooms observed with soap, paper towels and toilet paper. Bathroom hot water temperature maintained at 111.3 degrees F. LPA observed an isolation room which is equipped with PPE supplies. Daily calendar to include the participants, staff and activities posted in the hallway. LPA observed a weekly activities calendar. Facility has emergency disaster plan and infection control plan located on site. Emergency drills such as fire and earthquake drills are completed monthly. Facility has a first aid kit, flashlights and emergency lighting in the hallways available. 3 client files were reviewed. 1 of the client's appraisal/needs and services plan was last updated in March 2024 and the IPP was more than one year old. PD was advised regarding the regulations to ensure the appraisals or IPP is not more than one year old. Technical advisory provided. PD states they do not hold any client's medications and money. 3 staff files were reviewed and observed complete. 3 out of 3 staff obtains a first aid certification. Staff are provided at least 8 hours of documented annual training. Documents were requested to update the facility's file by Friday 10/10/2025: LIC500 and LIC308. No deficiencies were cited per California Code of Regulations, Title 22. Advisory note provided. This report was reviewed with Program Director, Alejandrino Espinoza and a copy of the report was provided.
