Facility Evaluation Report

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Abraham Longoria.

The purpose of the visit was to cite the facility for having two staff, S1 and S2, who were working at the facility without being associated to the facility.

S1 and S2 were both associated to Learning Services Corporation - Mesa House and Learning Services - South Valley Ranch #1, but not to Learning Services - South Valley Ranch #2, per Guardian employment records.

S1 began working at the facility on 08/05/2024 and was terminated from employment at the facility on 2/18/2026.

S2 began working at the facility on 03/20/2023 and was terminated from employment at the facility on 01/14/2026.

A deficiency was cited as per California Code of Regulations Title 22. See LIC809-D page for more information.

Two immediate $500 civil penalties were issued today, totaling $1,000. See LIC421BG for more information.
