Facility Evaluation Report
Licensing Program Analyst (LPA) Christine Kabariti arrived unannounced to conduct the facility's required - 1 year annual inspection. LPA met with Administrator, Abraham Longoria.
During visit, LPA toured Castillo House (South Valley Ranch #2) with the ADM to include the kitchen, dining room, resident bedrooms, bathrooms, living room, and exterior. Facility temperature maintained at 71 degrees F. All fire exit routes were free and clear of obstruction. Emergency exit routes facility sketch posted throughout the hallways. Exit doors contains a door alarm, if needed. Fire extinguishers last serviced on 01/07/2025. Carbon monoxide detector present. AED last serviced on 06/19/2025. Facility has a complete first aid kit and emergency response kit. Staff present are fingerprint cleared and associated to the facility.
Facility has at least 2 days worth of perishables and 7 days worth of non-perishable foods. Refrigerator maintained at 38 degrees F. Freezer temperature maintained at 0 degrees F.
LPA and ADM entered into rooms #1, 2, 3, 4, 6, 7, 8, and 9. Resident bedrooms equipped with beds, linens, adequate lighting, night stands, and closet space. 2 residents bedrooms observed with physician prescribed half rails. 2 residents with inhalation-assistive devices has a restricted health condition care plan and physician's order for the device.
