Facility Evaluation Report

Licensing Program Analyst (LPA) Marcella Tarin arrived at the facility unannounced to

conduct a case management – other visit. LPA met with Administrator (ADM) Abraham Longoria. LPA stated the purpose of the visit.

The purpose of the visit is to hand deliver immediate exclusion letters for three individuals (S1-S3) who the

Department determined engaged in conduct inimical.

ADM stated S1, S2 and S3 were not working in the facility. ADM states S1 and S2's last day of employment was 1/14/2026 and S3's last day of employment was 2/18/2026.

The immediate exclusion letters were handed to the ADM. The ADM was informed to remove S1, S2, S3 from having any contact with clients and S1, S2 and S3 were not allowed to be physically present in the facility. The ADM was advised to separate S1, S2, and S3 from the facility roster. LPA requested a copy of the updated LIC 500.

No deficiencies were cited per California Code of Regulations, Title 22.
