The inspector’s account

Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced 1 year required annual inspection and met with clinical director (CD) Lindsey Blakeley. Administrator Abraham Longoria was not available during the visit due to prior commitment.

The facility is licensed to serve adults 18-59 years, all maybe non-ambulatory and hospice waiver for five (5).

During visit, LPA observed the area is a compound composed of 3 buildings, Casa Castillo, Becker House, and the Therapy Center. LPA toured Becker House (South Valley Ranch #1) with CD to include the kitchen, dining room, resident bedrooms, bathrooms, common areas, and exterior.

LPA toured the exterior and indoor area. Emergency exits were free from obstructions and tripping hazards. Indoor temperature was within acceptable range of 72 to 74 °F. The kitchen was sanitary and organized; knives and chemicals were locked. Food supply met requirements (2 days perishable, 7 days non-perishable). Kitchen water temperature measured at 113.7°F. Bathroom water temperature ranged from 105°F to 113°F. Refrigerator temperature is at 38°F and freezer is at -9°F

Bathrooms had grab bars and non-skid mats; resident rooms had adequate storage. Medications were locked and inaccessible to residents; first aid kit was complete. Outdoor areas were free of hazards; laundry appliances were functional, and cleaning supplies were secured. Fire, smoke, and carbon monoxide systems were operational; hallways were clear and well-lit.
