Facility Evaluation Report
At 10:10am on 08/05/2026, Licensing Program Analyst (LPA) Jeffries arrived to the facility unannounced to conduct the facility annual inspection. LPA met with Administrator, Colodia Owens, announced who he is and the reason for the visit.
Administrator and LPA conducted a physical tour of the licensed day program facility. This day program is located in the Anex section of Saint Mary's Church. The Anex has two large open areas, an industrial kitchen, several storage closets, two bathrooms, one for male and one female use, an closed courtyard for outdoor activities for clients. There is a patio area at the front entrance also for client use. Day program accepts clients who do not require medications or PRN ( pro re nata, as needed) medications, as there is no medication storage. This day program provided lunch and snacks for clients. LPA noted that the water temperature in the facility was within regulation requirements of 105*-120*(f) in both bathrooms and kitchens, LPA noted that the fire extinguisher was primed and in the green located outside of the kitchen area. LPA noted that the day program has an internal sprinkler system with fire pull handles throughout the facility. Santa Barbara County Fire Department last passing inspection was conducted on 08/14/2025. LPA noted that the facility is clean and in good repair. LPA reviewed Emergency Disaster Plan, Infection control plan, staff and client files including but not limited to staff training, clearance, and medical screening. LPA noted the physical inspection did not revile any violations or citations at this time.
Administrator and LPA conducted a full review of the facility annual care tools modules. LPA noted that there were no violations or citations as a result of the full review of the annual care tool modules. LPA conducted staff and client interviews and conducted facility language survey (primary, English). LPA noted that there were no violations or citations as a result of the annual facility inspection.
