Facility Evaluation Report

Licensing Program Analyst (LPA), Albert Johnson conducted a case management visit to follow up on an incident that occurred on 06/13/25. LPA met with the facilities Manager and explained the purpose of today's visit.

R1 reported with to a staff of Person Center Services (PCS) of verbal abuse by R1's sister, including her sister screaming at R1 saying she is a retarded and stupid. R1 is currently expressing feelings of insecurity and fear in her own living environment. Emphasizing a desire for the sister to vacate the apartment for R1's own safety. Staff at PCS followed up and shared information with San Joaquin APS. A SOC 341 APS report was also filled out and submitted. PCS Staff reminded R1 to call 9-1-1 if they feels unsafe. CCL and R1's VMRC (Valley Mountain Regional Center) service coordinator and SLS(supportive living services) workers were also notified of the situation. R1 shared the plan to go to a friend's house or a hotel until a respite can be provided by VMRC if the situation should come to R1 needing to move immediately.

LPA interviewed R1 and determined that supports are in place for R1 if the need arises for R1 to relocate.
