The inspector’s account

LPA Johnson arrived to the facility unannounced to conduct a Case management visit regarding a self reported incident that occurred on 8/14/2025.

The incident report details that R1 had a seizure on 8/14/2025 at 12:48pm preparing to board the bus to go home. R1 has a history of seizures and the program has a plan to assist R1 when these events happen. The program's plan to support R1 with emergency services is based on the duration or the incessant repetitiveness of the seizure. The documented time of this seizure was less than five minutes and was not repetitive, therefore emergency services were not called.

Also observed during the visit was the adjusted time of departure for some clients. The program has an approved program plan for direct service hours of 8:30am to 2:30pm. LPA observed the program loading at 1:00pm. This was verified by R&D notes dated 11/14/2023 (approved) and noted again from R&D dated 4/12/2024 (change not approved). This c hanges in the plan of operations affects the services to clients and should be reported to the department for approval prior to implementation.
