The inspector’s account

On June 18, 2025, at approximately 0930 AM Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with administrator Mary McElwain and explained the purpose of the visit.

LPA and administrator toured the facility together to ensure the health and safety of clients who attend program. At the time of the inspection, no clients were present as they were all on field trips. Areas toured include but are not limited to common areas, two (2) bathrooms, kitchen, classrooms, storage areas and back yard. Four (4) staff and four (4) client files were reviewed.

There is a schedule of recreational activities planned for the clients. Each client is provided two copies of their own personal schedule for there locker and home use. Classrooms contained a plethora of items for clients to use including games, books, and art supplies.

The facility was observed to be at a comfortable temperature. All common areas and bathrooms were clean and in good repair. Fire extinguishers were fully charged and last inspected in July 2024. No pools/bodies of water are on premises. Last disaster drill was conducted in May 2025, which was a fire drill, the facility has been conducting fire drills monthly.

LPA requested the following documents in order to update the facility file:
