Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Fuller made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Maria Jimenez and discussed the purpose of the visit.
The facility is a two-story home with four client bedrooms with two being shared, two staff rooms, three bathrooms, living room, dining room, kitchen and attached garage. The facility appears clean, safe and sanitary. LPA observed smoke detectors/carbon monoxide to be operational. Client bedrooms had the required components and furnishings. There is one staff bedroom upstairs that is inaccessible to clients in care. The staff bedroom downstairs was unlocked with no items that should not be accessible to clients found. LPA observed the client restrooms to be stocked with toilet paper, paper towels and textured shower flooring. The water was tested at 102-102.3 degrees Fahrenheit. The extra supply of linens was found in the down stairs hall cupboard. The kitchen was clean and free of vermin. LPA observed a two day perishable and seven day non perishable food supply on hand. LPA observed the knives to be in a drawer located by the stove and made inaccessible to clients in care. LPA observed the centrally stored medication to be in a cabinet by the fridge and made inaccessible to clients in care. All appliances were found to be operational. LPA observed fire extinguishers charged and with a service date of May 11, 2026, throughout the facility. LPA observed a completed first aid kit in the closet under the stairs as well as the emergency food and water supply. The garage is used as extra facility storage such as activity supplies. Toxins and chemicals were observed to be locked in a cabinet in the garage and made inaccessible. The garage is also used as a staff office and laundry room. The backyard was observed to have no debris and obstructions with an outdoor shaded seating area for client enjoyment.
