Facility Evaluation Report
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to the facility to complete a required annual inspection. Upon arrival, LPA Haddadin was greeted and granted entry by Licensee (LC) Roslyn Gray. The purpose of the visit was explained. During the inspection, one client was present in care. LPA, accompanied by the Licensee, toured the interior and exterior areas of the facility. Required Department postings were observed to be displayed in a prominent location. The facility remains within its approved capacity. Food supplies were inspected and found to include at least a one-week supply of non-perishable items and a two-day supply of perishable items. Medications are centrally stored in a secured, locked cabinet located in the laundry area. LPA reviewed medication storage and verified that all medications were properly labeled and maintained inaccessible to clients in care. LPA inspected the bathroom and tested the hot water temperature, which measured between 117.6 and 118.9 degrees Fahrenheit. Bathrooms were equipped with soap, paper towels, and toilet paper. The facility maintains adequate cleaning, sanitizing, and hand hygiene supplies. During the kitchen inspection, all appliances were found to be in good working condition. Sharps and knives were locked in a cabinet located in the hallway near the bedrooms, ensuring they were inaccessible to clients. Smoke detectors and the carbon monoxide detector were tested and observed to be operational. LPA reviewed one client record, including Personal and Incidental (P&I) funds, and one staff record. All documentation was current and complete. The facility’s last emergency drill was conducted on August 4, 2025. Based on today’s inspection, no deficiencies were observed in the areas evaluated. An exit interview was conducted, and a copy of this report, along with the confidential names list, was provided to Licensee Roslyn Gray.
