Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit to the facility for the purpose of delivering an amended version of the complaint investigation report generated on February 20, 2025.
LPA met with Administrator (AD) Cris Festin and discussed the amendment.
An exit interview was conducted and a copy of this report was provided at the end of the inspection.
