Facility Evaluation Report

On today's date at 8:15 AM Licensing Program Analyst (LPA) Michael Tea made an attempted unannounced visit to facility to conduct an Annual Required inspection visit. LPA Tea called the facility number but the number is disconnected. LPA Tea then called the mobile phone number listed on file and attempted to leave a voice mail message regarding the annual inspection but the voice mail box was full. LPA then text the number with no response. While waiting, LPA discovered that facility was going under a change of ownership since November 2024. The facility is going through a change of ownership as Sapphire Guest Home. LPA than attempt to contact the new licensee and left a message for new licensee. Sapphire Guest Home passed Pre-Licensing visit conducted by LPA Alejandre on 07/28/2025. On 08/05/2025 Fire Inspector notified CAB regarding facility modifications were not approved by city and denied STD 850. CAB Specialist Celia Phomphachanh notated on 08/07/2025 that new facility licensee will withdraw application.

LPA Tea knocked on the door again. No one came out. LPA walked around the premises to check to see if anyone was on the premises.

This report will be reviewed and signed by facility when LPA has successfully complete contact.
