Facility Evaluation Report

On August 25, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. Upon arrival, LPA was informed by a tenant of the residence that the facility no longer operated as a Social Rehabilitation Facility. LPA was informed that as of the beginning of this year, the home was being used as Sober Living House called, Clean Pathway Behavioral Plan, through the County of Orange.

LPA made a telephone call to the Licensee (LI) on file, Ariana Crame. The LI confirmed with LPA via telephone that the Social Rehabilitation Facility was no longer operating. The LI stated that they were no longer associated to the facility and were unable to provide further details about the status of the facility.

LPA was unable to inspect the property since it was being used as a Sober Living House. LPA was unable to provide a copy of the report to anyone associated to the facility since there are no additional contacts on file. LPA will mail a copy of the report to the last address on file.
