Facility Evaluation Report

Licensing Program Analyst (LPA) Fred Arias conducted an unannounced visit to follow up on an incident report received by the Department on June 2, 2026. LPA was greeted and allowed entry by staff. LPA explained the reason for the visit. LPA toured the facility with staff. LPA observed clients engaged in activities in several areas of the facility, including an art room, gym area, computer room, and other open areas. LPA observed staff assisting and engaging with clients. LPA interviewed staff regarding the incident.

LPA did not observe any immediate health and safety concerns during today’s visit. No deficiencies cited at this time.

An exit interview was conducted and a copy of this report was provided.
