Licensing Program Analyst (LPA) made an unannounced visit for the purpose of conducting an annual inspection. Upon arrival LPA knocked on the door and rang the door bell several times. At 8:20 a.m. LPA placed a call to AD Lonnie Fischer and left a voicemail requesting a call back. At 8:29 a.m. LPA placed an additional call to AD and did not receive a response or an immediate call back. LPA waited at the facility but left after about 20 minutes with no response.
