Facility Evaluation Report
Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. made an unannounced case management visit as Licensee notified Community Care Licensing (CCL) on May 13, 2026, that the new fire clearance had been approved. LPA was greeted and granted entry by Administrator (AD) Michelle Nesbitt. LPA explained the purpose of the visit.
Fire Clearance approved by a fire inspector of Orange County Fire Authority on April 27, 2026. Special conditions noted, "No change to capacity. Increased the facility from 5 bedroom 2 bathroom RCFE with 6 non-ambulatory residents to a 6 bedroom 3 bathroom RCFE with capacity for no more than 6 non-ambulatory residents."
The facility is a one story house with six resident bedrooms, four bathrooms, two of which are shared bathrooms, one staff bedroom, a kitchen, a living room and an attached garage. LPA Ramirez observed a shaded patio area with furniture, and the grounds were free of any hazards.
LPA also conducted a Health and Safety inspection and observed the facility to have electricity, water, and gas. Appliances, including: electric stove, microwave, washer, and dryer were observed to be operational. Fire extinguisher was observed to be fully charged with a service tag dated April 8, 2026. Resident bedrooms were observed to have the required furnishings. The kitchen was observed to be clean and organized, and a 7-day supply of non-perishable food was observed. Medications will be stored and locked in a closet by the residents' bedroom hallway.
