Facility Evaluation Report

Licensing Program Analyst (LPA) Ruth Martinez conducted an announced visit to the facility for purpose of a pre-licensing evaluation. LPA arrived at facility was greeted at the door by Jill Bonnin, Administrator and Tony Duarte, Area Director and was granted entry.

An initial application to operate an Adult Residential Facility (ARF), for (4) capacity, (0) ambulatory, (4) non-ambulatory, and (0) bedridden clients was submitted to CCL on 03/18/2022.

The facility is a one story house with an attached garage with 4 client bedrooms, 2 full bathrooms, 1 Living room, a kitchen. The client’s bedrooms are spacious and will easily accommodate the client’s furnishings. There is a large back yard with an exit walkway on each side of the house. There is covered seating for the clients in the backyard.

Central air/heating system installed with a central panel to control entire house.

Bedrooms are for 4 non-ambulatory clients. Bedrooms will accommodate 4 clients with all bedrooms being private. Bedrooms 1 has a full bathroom.
