The inspector’s account

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit for the purpose of conducting a required annual visit. LPA was greeted and granted entry by staff and later joined Rey-Philp Clinical Director (CD).

LPA along with CD toured the interior and exterior of the facility and observed the following:

The facility is a two-story home with three client bedrooms, two bathrooms, kitchen, dining room, living room, therapy room, med room, backyard, and attached two-car garage.

Two bedrooms, one bathroom and the med room are located on the second floor. At the time of visit, facility had 6 clients in care. LPA observed some clients to be participating in group therapy and others in their perspective rooms. Facility appears clean, sanitary, and organized.

All clients’ rooms had the required elements, including bed, chair, closet space and ample lighting. Restrooms are stocked with soap and paper towels and have hand washing postings. Hot water measured at 114.4 degrees Fahrenheit in both bathrooms.

LPA observed facility has one week of non-perishables foods and two days of perishables foods available. LPA reviewed medication records for two clients and insured medications are centrally stored in the medication room located on the second level locked and inaccessible to clients in care. LPA observed that toxic chemicals, cleaning solutions and disinfectants are stored locked in a cabinet located in the garage.

Carbon Monoxide/Smoke Detectors were tested and operational. Fire Extinguishers were fully charged with Service tags indicated the extinguishers were last serviced on February 11ith, 2024. Fire drills conducted every month and LPA verified last Fire Drill was conducted on November 6th, 2024.

The backyard has one shaded seating area and ample space for activities. Exit gate is unlocked and self-latching no obstructions or obstacles noted.
