The inspector’s account

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Compliance Officer Kerri Bamper, Clinical Director Sadaf Forouhar and Residential Supervisor Willie Tucker and explained the reason for the visit. Facility is a two story home with 3 bedrooms upstairs, family room off of the kitchen with a screened fireplace, kitchen, dining room, medication room (downstairs), therapy room (downstairs) living room, 3 bathrooms and an attached 3 car garage. Facility is licensed as a Social Rehabilitation Faciltity for 6 ambulatory clients. Facility is a transitional home, not a short term crisis home. LPA and staff toured the facility. The garage is kept locked and used for storage. LPA observed a 3 day emergency supply of food and water stored in the garage. LPA observed all chemicals and cleaning supplies are kept in locked cabinet in the garage. LPA inspected the attic space above the garage. LPA observed the attic space is used for storage. LPA observed all client bedrooms have the required furnishings and bed linens. LPA observed an emergency evacuation chair at the top of the staircase. The fire extinguisher at the top of the staircase and in the fire extinguisher in the kitchen are fully charged. LPA observed clean linens stored in the second floor pantry. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed the fireplace in the family room is screened. LPA observed all 3 bathrooms are clean and operational. Hot water measured 110.4 to 110.6 degrees Fahrenheit in all 3 bathrooms. No obstacles or hazards observed inside of the facility. LPA observed all medications and sharp objects are kept locked in the medication room. LPA and staff toured the backyard. No bodies of water observed. There is a covered patio for clients to sit outside. The exit gate is latched and self closing. No obstacles or hazards observed in the backyard. The last emergency drill was conducted on July 16, 2026. Smoke detectors/carbon monoxide detectors tested operational. LPA reviewed 2 staff files, no discrepancies observed.
