Facility Evaluation Report
Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Clinical Director (CD) David Asch and explained the reason for the visit. Facility is a two story home with 3 bedrooms upstairs, living room off of the kitchen, kitchen, dining room, great room, 3 bathrooms and an attached 3 car garage. The garage is kept locked and used for storage. LPA and staff toured the facility. LPA observed a 3 day emergency supply of food and water stored in the garage. LPA observed all client bedrooms have the required furnishings and bed linens. LPA observed and emergency evacuation chair at the top of the staircase. LPA observed clean linens stored in the second floor pantry. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed the fireplace in the living room is screened. LPA observed all 3 bathrooms are clean and operational. Hot water measured 113.5 degrees Fahrenheit in all 3 bathrooms. No obstacles or hazards observed inside of the facility. LPA and staff toured the backyard. No bodies of water observed. There is a covered patio for clients to sit outside. The exit gate is latched and self closing. No obstacles or hazards observed in the backyard. LPA observed all 3 fire extinguishers are fully charged. The last emergency drill was conducted on July 16, 2025. Smoke detectors/carbon monoxide detectors tested operational. LPA reviewed staff files, both staff members are background cleared and associated to the facility. Both staff members have CPR/First Aid training and the required 20 hours of annual training. LPA reviewed 6 client records and medications, no discrepancies observed. LPA inspected the first aid kit, the first aid kit has all the required elements. Facility has a dedicated internet device for client use only. LPA consulted with the CD regarding reporting requirements. No deficiencies observed during the visit. An exit interview was conducted and a copy of the report provided.
