The inspector’s account

On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry by staff and explained the reason for the visit. Licensee Michael Rillera and Administrator Armando Flores arrived shortly after.

Facility is licensed for 2 ambulatory and 3 non-ambulatory clients and the facility currently has 3 clients. The home consists of four client bedrooms, two shared hall bathrooms, client restroom, one staff room, living room, dining room, and kitchen.

During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors and testing hot water temperature in three bathrooms and kitchen sink. The hot water temperature measured 105.4 and 115.1 degrees Fahrenheit and all smoke detectors were operational. The facility conducted an emergency drill on 07/15/2025. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. LPA observed emergency food and water in hallway closet.

LPA reviewed five out of five staff training and fingerprint records and conducted a complete review of three out of three clients records. LPA confirmed that Administrator Armando Flores has a current administrator certificate which expires on 04/18/2026 and Licensee Michael's administrator certificate expires 1/30/2026.

No deficiencies noted, an exit interview was conducted and a copy of this report was provided.
