The inspector’s account

This unannounced Case Management – Incident inspection is being conducted by Licensing Program Analyst (LPA) Sean Haddad for the purpose of following on self-reported incident reports received in the Orange County Regional Office (OCRO) on 08/28/23 regarding multiple incidents of Client #1 (C1) eloping from the facility. LPA met with Staff #1 (S1) Sara Bashash and discussed the purpose of the inspection. Program Director (PD) Melody Fathi arrived during the inspection.

The incident reports state the following: On 08/24/23 at approximately 2:35PM, C1 returned to the facility after eloping and stated they had been at the beach and were returned by their family member. On 08/25/23 at approximately 5:34PM, C1 left the facility, staff attempted to follow but were unsuccessful, and C1 was returned to the facility by police at 3:44PM. On 08/26/23 at approximately 11:00AM, C1 became aggressive, threatened staff and attempted to spit on staff, and stormed out of the facility.

During the inspection, LPA inspected the facility, observed 4 staff and 3 clients present, conducted health and safety checks on the clients, and observed no health and safety issues.

There were no health and safety concerns observed in the areas inspected. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report was discussed with and provided to facility representative.
